BY MARLENE PITURRO ardiovascular disease is the leading cause of death in women as well as men. More than twice as many women die from CVD as from all forms of cancer combined. Heart disease doesn't discriminate when it strikes. When it comes to diagnosing and treating CVD, though, there are measurable differences between the sexes based on myriad factors-- attitude, hormonal and similar biological factors, risk factors, and how male and female bodies respond to common diagnostic tests and surgeries. The emergency room highlights CVD gender differences. Thanks to thrombolytics, clot-busting drugs such as TPA and streptokinase, heart attack survival rates have greatly improved in the past ten years, but more so for men than women. A Washington state study of 1, 078 men and women showed that 30% of women arrived at the E.R. too late for thrombolytic therapy, and only 55% of eligible women versus 78% of men received the necessary drugs. Why the differential treatment? It's unclear. Clot-busting drugs work best when administered within one hour of a heart attack and women delay getting to the E.R. longer than do men. Methods: 53 patients with schizophrenia and 48 healthy comparison subjects were studied. Informed consent was obtained from all subjects. High-resolution 3dimensional MRI was acquired and volumes of anterior parts of the internal capsule, caudate nucleus, and lentiform nucleus were measured. In addition, white matter concentration was compared using the voxel-based morphometry VBM ; . Results: Compared to the controls, the patients had significantly decreased volumes in the bilateral anterior internal capsule, although volumes of the caudate or lentiform nuclei were not different between the groups. VBM also revealed reduction in white matter concentration of the bilateral internal capsule in the patients. Conclusions: The volume reduction found in the anterior internal capsule further supports the hypothesis of abnormal fronto-thalamic connectivity in schizophrenia. 32.

Several possible actions have been examined. In the certification programme, it was already decided that the energy efficiency EER and COP ; replace the power input as the certified characteristics. A voluntary classification system has been implemented although for such large equipment no physical labels will be fixed. However classes will increase awareness of users that the energy efficiency is an important issue but also the manufacturers when they design new equipment. With a classification it may be easier to eliminate, like for air conditioners, the low efficiency products. The effect on the market has already been identified as some users simplify their request asking Class A products. The use of chillers and the real annual energy consumption has been largely discussed. As the part load efficiency has a very strong impact on consumption, in the SAVE study EECCAC Energy Efficiency and Certification of Central Air Conditioners - this issue has been treated exhaustively. This study was initiated by Eurovent Cecomaf and manufacturers are actively participating. An integrated energy efficiency index called ESEER similar to IPLV Integrated Part Load Value ; developed in the USA and used by ARI, was established for the European conditions. The study was finalised in 2004 and experimental application started in 2005. Part load certification will be implemented in the Eurovent certification in 2006. The WG 6C Air Handling Units" after finalising the Life Cycle Cost project has realised that the most important part of this cost is the energy consumption. Although other factors, like maintenance, are also very important, the best means to decrease the life cycle cost of an air handling units is to design and operate in the optimum way with respect to energy consumption. A Eurovent Recommendation concerning assessment of the energy consumption of air handling units has been prepared. This document provides the detailed analysis of all components: fans, cooling and heating coils, heat recovery units. Recommendation was published in May 2005. The Eurovent WG 4 B Air Filters has found, during the work on Life Cycle Cost project, that energy consumption represents the largest part of the cost. Although the filters are passive elements and do not consume energy directly, the pressure drop they introduce in the flow is related to fan consumption. Selection of filters and their replacement should be made taking into account not only their primary purpose cleaning the air ; but also the possible energy saving. The relevant Eurovent Recommendation concerning calculation of Life cycle Cost of Air Filters was revised in 2004. The WG 14 Refrigerated Display Cabinets" has been working on annual energy consumption. The Group has been very much involved in the action undertaken by public authorities in the United Kingdom where Market Transformation Programme is aimed to increase the use of more efficient products. In order to implement the Eurovent Certification as the single data base for all Europe it will be necessary to highlight more efficient display cabinets. Selection of LBEs Exelon Proposal Exelon proposes to use a PRA to provide a logical and structured method to evaluate the overall safety characteristics of the PBMR plant. This will be accomplished by systematically enumerating a sufficiently complete set of accident scenarios and by assessing the frequencies and consequences of the scenarios individually and in the aggregate to predict the overall risk profile. A PRA is seen by Exelon as the best available safety analysis method for showing how the dependencies and interactions among SSCs, human operators, and the internal and external plant hazards that may perturb operation of the plant and produce an accident. The quantification of both frequencies and consequences must address uncertainties because it is understood that the calculation of risk is affected by uncertainties associated with the potential occurrence of rare events. These quantifications provide an objective means of comparing the likelihood and consequences of different scenarios and of comparing the assessed level of safety against the TLRC. Exelon seeks to use the PRA to select families of events based upon expected frequencies and consequences ; which will become the LBEs for the PBMR. The families will consist of events of similar characteristics initiator, end states ; . Exelon notes that families of events may have significant uncertainties in the estimate of their frequencies. The consideration of these uncertainties is necessary to assure that all events will be assessed against the appropriate criteria. An additional factor is placed on the mean frequency to assure that event families falling just above or below a region are evaluated in the most stringent manner. Anticipated Operational Occurrences Region Exelon identifies AOOs as those conditions of normal operation which are expected to occur one or more times during the life of the plant. Using a licensing basis design lifetime of 40 years yields a lower boundary for the AOO region of 2.5 x 10-2 per plant year. For this region, Exelon views 10 CFR Part 50, Appendix I as providing the applicable criterion, as it specifies the numerical guides on design objectives for assuring that releases of radioactive material to unrestricted areas during normal reactor operations, including AOOs, are maintained ALARA.

The phospholipid PIP2 reduces the maximal block produced by saturating concentrations of tolbutamide 28 ; and glibenclamide 29 ; . It also increases the IC50 of the drug-sensitive fraction of channels. The effects of PIP2 are attenuated by the mutation R176A in Kir6.2 29 ; , which is believed to reduce PIP2 binding to this subunit. Furthermore, negatively charged lipids reduce glibenclamide block with similar potency, whereas neutral or weakly charged lipids have no effect. As described above, the effects of PIP2 on sulfonylurea sensitivity can be largely accounted for by the increase in channel PO produced by the phospholipid. However, it is also possible that PIP2 like the NH2-terminal deletions ; has additional effects on sulfonylurea block that are masked by the effect on the channel PO. The amount of PIP2 in the membrane can vary as a result of altered phospholipid metabolism. Thus PIP2 is expected to fall in response to ischemia because falling ATP levels reduce its generation from PIP by phosphorylation ; and increase when phosphatidylinositol kinases are stimulated. This means that PIP2 regulation of glibenclamide block will also be subject to regulation.
After identifying a possible mental disorder in a woman during pregnancy or the postnatal period, further assessment should be considered, in consultation with colleagues if necessary. If the healthcare professional or the woman has significant concerns, the woman should normally be referred for further assessment to her GP. If the woman has, or is suspected to have, a severe mental illness for example, bipolar disorder or schizophrenia ; , she should be referred to a specialist mental health service, including, if appropriate, a specialist perinatal mental health service. This should be discussed with the woman and preferably with her GP. The woman's GP should be informed in all cases in which a possible current mental disorder or a history of significant mental disorder is detected, even if no further assessment or referral is made.
The science is overwhelming. Climate change is happening now, and greenhouse gases GHGs ; due to human activity are a leading cause. In the United States, the power industry accounts for 35% of all GHGs. Some in Washington are developing legislation to limit GHG emissions from all sources, and we strongly support those efforts. But as the nation's largest electric and gas utility, and as a member of the U.S. EPA's Climate Leaders Program, we are acting now. As you read this, we are on track to exceed our goal of reducing our GHG emissions by 8% from 2001 levels by the end of this year. But that's not enough. By 2020, we are committed to reducing, offsetting or displacing more than 15 million tonnes of GHGs per year. This would be more than our current carbon footprint and is equivalent to having nearly 3 million fewer cars on our roads and highways. We will do this by cutting emissions wherever we can: in our generating plants, in our facilities, in our electric markets and in the communities we serve.
